Diabetes Management

Amtan has developed a comprehensive Diabetes Care management plan based on years of experience, built on a foundation of Australian medical best practices.

Diabetes is a complex condition and extra help is often needed for patients to reach their health goals. The continuity of patient care and monitoring will help support a Diabetes management program, facilitated by the experts at Amtan Medical.

Heart Health Management

With evolving modern lifestyles, Heart Health is one of the most critical focus areas for medical practioners. Amtan has been leading the way in Heart Health Care, including being the first General Practice in Australia to trial the next generation portable heart health scanning technology. Our senior medical team regularly attends and presents at various heart health focused conferences, and brings this wealth of experience to the co mmunity with strong long term patient heart health care.

Cancer Screening Management

With early detection some cancers can be detected at an early stage when treatment is likely to be more effective. Tests have been developed that can detect these cancers well before any symptoms are present. The aim of screening programs is to pick up very early cancers in healthy individuals, who do not have symptoms, or in some cases to prevent cancer.

Chronic Disease Management

If you are living with a chronic disease, we offer support to manage the ongoing symptoms and day-to-day challenges. Our self-management programs will support and encourage you to maintain long-term health and wellbeing. Amtan has been at the forefront of ongoing chronic disease management, including the care of long term conditions such as asthma, arthritis, cancer, diabetes, coronary heart disease, osteoporosis, dementia or chronic obstructive pulmonary disease (COPD).

 

Telehealth

Telehealth connects patients to vital health care services through videoconferencing, remote monitoring, electronic consults and wireless communications. By increasing access to physicians and specialists, telehealth helps ensure patients receive the right care, at the right place, at the right time. Training is available to support clinicians to deliver telehealth.

Primary Prevention

Focuses on reducing risk factors to prevent a disease or disorder before it arises.

Secondary Prevention

Focusses on the early detection and best practice management of disease or disorder to reduce deterioration and long-term effects.

Tertiary Prevention

Focuses on reducing harms in people with a disease or disorder and minimising their functional impairment.

Quaternary Prevention

Focuses on reducing harms caused by medical interventions for a disease or a disorder.
